A 2022 systematic review in BMC Geriatrics found medium-chain triglycerides may improve memory in non-demented older adults, based on a small number of randomized trials. A 2023 randomized controlled trial in Physiology and Behavior found an MCT blend improved cognitive performance and reduced the cognitive decline associated with prolonged exercise in young healthy adults. Both are narrow, short and use isolated MCT rather than a butter-and-oil coffee.

The honest caveat: no study has tested Bulletproof coffee, so the crash-free-energy claim is untested rather than disproven. Meanwhile a 2020 meta-analysis in Circulation found coconut oil significantly increased LDL cholesterol by 10.47 mg/dL (95% CI 3.01 to 17.94) compared with non-tropical vegetable oils, and butter contributes further saturated fat. Evidence does not support this drink for weight loss, longevity, or as a nutritionally adequate breakfast, and anyone with raised LDL or a family history of heart disease should treat it accordingly.
